Med reconcilliation…one of my fav topics…


that is always a topic of debate no matter where you go. What does it mean? What does any policy to address it really mean? This is such a nebulous area of patient care and it is also one of the most controversial areas that I run across regularly. I’m not a provider and I have read the rules but as a patient I think that I want my physicians to know everything I am taking and to consider it when they review my chart regardless of their specialty. I have my own health challenges that are firmly under control but my diligence with meds and interactions and updating my doctors is a major key to that success.
